About the Position:
The Team Lead is responsible for the overall leadership, coordination, and quality delivery of a multisectoral humanitarian and development program in East Hararghe and Borena. The role ensures that interventions across sectors such as food security and livelihoods, WASH, health, nutrition, and protection are well planned, integrated, and implemented in line with organizational standards, donor requirements, and community needs.
The Team Lead provides day-to-day management of field teams, oversees budgets and resources, and ensures strong monitoring and accountability systems. The position plays a key role in coordinating with government authorities, partners, and other stakeholders, and in representing the organization at zonal and woreda levels.
Working closely with the Area Manager and technical teams, the Team Lead ensures that programs are responsive to changing contexts, achieve their intended results, and contribute to both immediate humanitarian needs and longer-term development outcomes. The role also supports proposal development, reporting, risk management, and staff capacity building to maintain high program quality and operational efficiency.
Job Purpose
The Medical Referral Support Officer is responsible for coordinating and facilitating timely medical referrals for patients requiring specialized diagnostic, treatment, or consultation services. The role ensures effective communication, proper documentation, and follow-up of referral cases, particularly for vulnerable individuals, while maintaining strong collaboration with healthcare providers and external service partners.
KEY AREAS OF ACCOUNTABILITY/DUTIES
1. Key Responsibilities and Duties
1. Referral Coordination
Coordinate referrals for patients requiring specialized services, diagnostic tests, or consultations with external healthcare facilities or specialists.
Receive referrals related to vulnerable individuals experiencing barriers in accessing healthcare and support them in connecting to appropriate health services.
Facilitate referrals to primary healthcare centers and specialized health services such as hospital admissions, psychiatric care, severe malnutrition treatment, emergency obstetric care, and other required medical services.
2. Case Follow-up and Patient Support
Follow up with referred patients to ensure they access the necessary services and receive appropriate care.
Support patients in scheduling appointments, obtaining diagnostic results, and addressing barriers to care.
Provide assistance to beneficiaries throughout the referral process while respecting confidentiality and patients’ rights.
3. Communication and Coordination
Communicate effectively with patients, families, and healthcare providers regarding referral needs, procedures, and follow-up care instructions.
Collaborate closely with internal healthcare teams, including physicians, nurses, and other departments, to ensure continuity of care.
Establish and maintain strong working relationships with external healthcare facilities, specialists, and community resources to facilitate timely referrals.
Daily patient report to Area manager and Project manager to Area office, HO, and RRS and UNHCR public health team.
4. Documentation and Reporting
Maintain accurate and complete documentation of referral requests, approvals, and outcomes in patient records and referral tracking systems.
Ensure timely reporting of activities to the supervisor using RAIS for individual case management and maintain Excel tracking sheets for referral and environmental health event follow-up.
Ensure all referral documentation complies with confidentiality and data protection standards.
All documentation from refugee camp, Area office by RRS, UNHCR and the patient documentation showing refugee status should be documented.
5. Patient Awareness and Health Education
Provide informal one-on-one awareness sessions to referred beneficiaries on key health topics when needed, including non-communicable diseases (NCDs), Infant and Young Child Feeding (IYCF), waterborne diseases, and hygiene practices.
6. Administrative and Logistics Support
Raise Purchase Requests (PR) for patient accommodation, flights, or transportation arrangements when referrals require travel.
Facilitate transportation and logistical arrangements for patients requiring referral services.
7. Other Duties
Perform other related duties as assigned by the supervisor to support the effective implementation of the health program.

Bachelor’s degree in Nursing or Health Officer from a recognized institution.
Must have a valid professional license and Certificate of Competency (COC) from the relevant regulatory body.
Minimum of four (4) years of relevant professional experience in recognized private or government specialized/referral hospitals, or experience working with non-governmental organizations (NGOs) in health programs.
Experience in patient referral systems, case management, or clinical service coordination is an added advantage.
Strong communication and interpersonal skills for effective interaction with patients, families, and healthcare providers.
Ability to coordinate referral services and manage patient follow-up effectively.
Good documentation, reporting, and data management skills, including use of referral tracking systems and Excel.
Strong understanding of healthcare service delivery systems and referral pathways.
Ability to work with vulnerable populations while maintaining confidentiality and ethical standards.
Strong organizational, problem-solving, and coordination skills.
Ability to work independently and as part of a multidisciplinary team.
